Both photos are same patient, in different times... Intestine is entering in scrotum and you can ear peristaltisme in scrotum. You can also feel the peristaltisme if you catch scrotum in hand... Intestine wille escape of the hand if you press scrotum...

Nothing to do for HIV dying patient. When rich that level, reduction is simply useless... But if you see signs of occlusion/peritonitis/strangle intestine... good luck with??? Antibiotics + soft reduction???... It is the job of a surgeon... Mild hernia is more dangerous than big hernia.
